What services does your clinic provide?

Therapy for Kids, Inc. provides pediatric physical therapy evaluations and treatment sessions.

How do I schedule an evaluation/appointment for my child?

What types of patients do you treat?

Therapy for Kids, Inc. treats children 0-18 years old with orthopedic, neurological, and developmental issues with a focus on children 0-3 years old with developmental delays and torticollis/plagiocephaly.

How long is a physical therapy evaluation or treatment session?

Both evaluations and treatment sessions are 1 hour long.

Is a prescription required for an evaluation/appointment?

Illinois is a direct access state so you do not need a prescription in order to get a physical therapy evaluation. However, if your child requires follow-up treatment and we are billing your insurance, we require a prescription from your child's pediatrician to keep on file.

Do you take insurance?

The only insurance Therapy for Kids, Inc. accepts is BCBS PPO and we are in-network with BCBS PPO. If you do not have BCBS PPO we do offer a private pay rate of $210 for an evaluation and $140 for follow-up sessions. We can also provide a superbill for you to submit to your insurance company for reimbursement.

Therapy for Kids, Inc. is located at 1512 N Fremont St, Suite 101, Chicago, IL 60642 in Lincoln Park.

Monday – Thursday 9:00am – 5:00pm

Where can I park?

There is street parking in the area and Therapy for Kids, Inc. has one spot in the pay-to-park lot located at 1529 N. Kingsburg St. You will need to get a parking pass from your therapist that you will place on your dashboard in order to park in this lot free of charge.
